Medication Reviews
Patients on repeat medication will be asked to see a Our Clinical Pharmacist Mr S Kumar, Nurse Practitioner or Practice Nurse at least once a year to review their medications.
The practice runs a birthday recall system for chronic conditions so your review will be on or around the month of your birthday. Don't worry if your review falls during another month, this is just a general guide to the recall.
If you have been notified that you need a review, please inform reception that you need a review and they will book with the appropriate clinician, not doing so may lead to inappropriate appointments and possibly delays to further prescriptions.
Medication Monitoring
Certain medications require regular monitoring i.e blood tests, blood pressure. These include, but are not limited to: Warfarin, Methotrexate, Lithium, Azathioprine, Levothyroxine, Leflunomide.
Please make sure your monitoring requirements are up to date before ordering your repeat medication, not doing so may cause a delay to your prescriptions or a reduced amount may be given until your monitoring is up to date.
